𝗜𝗥𝗔𝗣: 𝗧𝗵𝗲 𝗔𝗻𝘁𝗶-𝗜𝗻𝗳𝗹𝗮𝗺𝗺𝗮𝘁𝗼𝗿𝘆 𝗘𝘃𝗲𝗿𝘆𝗼𝗻𝗲’𝘀 𝗧𝗮𝗹𝗸𝗶𝗻𝗴 𝗔𝗯𝗼𝘂𝘁 (𝗔𝗴𝗮𝗶𝗻)

If you’ve been around orthobiologics, you’ve heard of 𝗜𝗥𝗔𝗣 — 𝗜𝗻𝘁𝗲𝗿𝗹𝗲𝘂𝗸𝗶𝗻-𝟭 𝗥𝗲𝗰𝗲𝗽𝘁𝗼𝗿 𝗔𝗻𝘁𝗮𝗴𝗼𝗻𝗶𝘀𝘁 𝗣𝗿𝗼𝘁𝗲𝗶𝗻.

Its job: block 𝗜𝗟-𝟭, the cytokine that drives joint inflammation and cartilage loss.

When IL-1 is blocked by 𝗜𝗟-𝟭𝗥𝗮, inflammation cools — often dramatically.

𝗪𝗵𝗲𝗿𝗲 𝗜𝘁 𝗦𝘁𝗮𝗿𝘁𝗲𝗱

In the early 2000s, 𝗗𝗿. 𝗣𝗲𝘁𝗲𝗿 𝗪𝗲𝗵𝗹𝗶𝗻𝗴 (Germany) discovered that incubating blood with treated glass beads triggered white cells to release 𝟮𝟬–𝟱𝟬× 𝗺𝗼𝗿𝗲 𝗜𝗟-𝟭𝗥𝗮.

That conditioned serum became 𝗢𝗿𝘁𝗵𝗼𝗸𝗶𝗻𝗲® / 𝗥𝗲𝗴𝗲𝗻𝗼𝗸𝗶𝗻𝗲®, the first IRAP-type therapy.

𝗥𝗲𝗴𝘂𝗹𝗮𝘁𝗼𝗿𝘆 𝗥𝗲𝗮𝗹𝗶𝘁𝘆 (𝗨.𝗦.)

From what we can tell, those original bead-based ACS/IRAP systems are 𝗻𝗼𝘁 𝗙𝗗𝗔-𝗰𝗹𝗲𝗮𝗿𝗲𝗱 𝗼𝗿 𝗮𝗽𝗽𝗿𝗼𝘃𝗲𝗱 and remain primarily available overseas.

Some newer “𝗔𝗣𝗦” systems aim for a same-day protein-rich plasma using 𝗰𝗹𝗼𝘀𝗲𝗱 𝗣𝗥𝗣-𝗹𝗶𝗸𝗲 𝗱𝗲𝘃𝗶𝗰𝗲𝘀, but intra-articular APS use for OA still appears 𝗶𝗻𝘃𝗲𝘀𝘁𝗶𝗴𝗮𝘁𝗶𝗼𝗻𝗮𝗹 𝗶𝗻 𝘁𝗵𝗲 𝗨.𝗦.

𝗪𝗵𝘆 𝗜𝘁’𝘀 𝗕𝗮𝗰𝗸 𝗶𝗻 𝘁𝗵𝗲 𝗦𝗽𝗼𝘁𝗹𝗶𝗴𝗵𝘁

Because 𝗜𝗟-𝟭𝗥𝗮 𝗶𝘀𝗻’𝘁 𝗷𝘂𝘀𝘁 𝗮𝗻𝗼𝘁𝗵𝗲𝗿 𝗴𝗿𝗼𝘄𝘁𝗵 𝗳𝗮𝗰𝘁𝗼𝗿 — 𝗶𝘁’𝘀 𝗮 𝗺𝗮𝘀𝘁𝗲𝗿 𝘀𝘄𝗶𝘁𝗰𝗵 in the inflammatory cascade.

High IL-1Ra levels have been linked with improved 𝗽𝗮𝗶𝗻 𝗮𝗻𝗱 𝗳𝘂𝗻𝗰𝘁𝗶𝗼𝗻 𝘀𝗰𝗼𝗿𝗲𝘀 𝗶𝗻 𝗸𝗻𝗲𝗲 𝗢𝗔 and protective effects on 𝗰𝗮𝗿𝘁𝗶𝗹𝗮𝗴𝗲 𝗮𝗻𝗱 𝘀𝘆𝗻𝗼𝘃𝗶𝘂𝗺 in preclinical models.

Here’s the interesting twist:

Dr. 𝗗𝗼𝗻 𝗕𝘂𝗳𝗼𝗿𝗱 and others point out that 𝗯𝗼𝗻𝗲 𝗺𝗮𝗿𝗿𝗼𝘄 𝗽𝗹𝗮𝘀𝗺𝗮 𝗮𝗹𝗿𝗲𝗮𝗱𝘆 𝗰𝗼𝗻𝘁𝗮𝗶𝗻𝘀 ~𝟮𝟬× 𝗵𝗶𝗴𝗵𝗲𝗿 𝗯𝗮𝘀𝗲𝗹𝗶𝗻𝗲 𝗜𝗟-𝟭𝗥𝗮 than peripheral blood.

So, concentrating and reducing plasma volume from 𝗯𝗼𝗻𝗲 𝗺𝗮𝗿𝗿𝗼𝘄 𝗮𝘀𝗽𝗶𝗿𝗮𝘁𝗲 could theoretically reach IRAP-like IL-1Ra levels —

all within 𝘀𝗮𝗺𝗲-𝗱𝗮𝘆, 𝗺𝗶𝗻𝗶𝗺𝗮𝗹𝗹𝘆 𝗺𝗮𝗻𝗶𝗽𝘂𝗹𝗮𝘁𝗲𝗱 procedures.

Not an “approved IRAP kit” — but biologically, it makes sense.

𝗕𝗶𝗴 𝗣𝗶𝗰𝘁𝘂𝗿𝗲

IRAP and its cousins (ACS, APS, BMAC-derived plasma) represent a shift:

𝘍𝘳𝘰𝘮 𝘨𝘳𝘰𝘸𝘵𝘩-𝘧𝘢𝘤𝘵𝘰𝘳-𝘥𝘳𝘪𝘷𝘦𝘯 𝘳𝘦𝘨𝘦𝘯𝘦𝘳𝘢𝘵𝘪𝘰𝘯 → 𝘵𝘰 𝘤𝘺𝘵𝘰𝘬𝘪𝘯𝘦-𝘥𝘳𝘪𝘷𝘦𝘯 immunomodulation.

Cool the fire first. Then let the joint heal.
